To comply with USPS regulations, clients must submit a notarized USPS Form 1583 along with two valid forms of identification before service begins. Service will not be activated without completed documentation.


✅ Primary ID

  • Primary ID must show a clear photograph and valid name. Primary acceptable forms of ID include:

    • U.S. state-issued driver’s license

    • U.S. state non-driver ID card

    • U.S. passport or passport card

    • Foreign passport

    • U.S. permanent resident card (Green Card)

    • Certificate of citizenship or naturalization

    • Uniformed Services or Armed Forces ID


✅ Secondary ID

  • Secondary ID must clearly include your name and residential street address (P.O. boxes don’t count). Secondary acceptable forms of ID include:

    • State-issued driver's license or ID card (if not used as the primary ID)

    • Lease, mortgage, or deed of trust

    • Voter registration card

    • Vehicle registration card

    • Home or Vehicle Insurance Policy


🚫 Unacceptable Forms

These do not meet USPS requirements (primary or secondary):

  • Social Security cards

  • Birth certificates

  • Credit cards

  • Utility bills

  • Bank statements


You cannot use the same document to satisfy both requirements—e.g., if you use your driver’s license as the photo ID, you’ll need a different document for address proof.
